INSOMNIA: WHY CAN'T I SLEEP?

If you're here, it's very likely because you're unable to sleep right now. Insomnia is a very common sleep disorder worldwide, and especially in the United States, it is considered an epidemic-scale problem.

According to data from the National Sleep Foundation, 30% of Americans experience sleep disruptions, and another 10% struggle with insomnia. Insomnia can involve the inability to fall asleep (sleep onset insomnia) or the inability to stay asleep (sleep maintenance insomnia).

So, answering the question WHY DO I HAVE INSOMNIA? The reality is that there are many risk factors that lead to this bothersome condition. It's your responsibility to identify these factors and take action starting today.

RISK FACTORS

  1. Gender and Hormones:
    Yes, gender. Women are more likely to have insomnia problems than men due to hormonal changes during menstruation, pregnancy, and the bothersome menopause. Sex hormones such as estrogen, progesterone, and testosterone can significantly affect sleep, particularly in women.
    Up to 61% of postmenopausal women report symptoms of insomnia.
    Sleep disorders have been linked to lower testosterone levels in both men and women. While obesity and abdominal fat are associated with lower testosterone levels in men, low testosterone levels, in turn, can promote obesity.

  2. Psychological Health:
    Insomnia can be both a symptom and a contributor to many mental health problems, such as anxiety, depression, schizophrenia, attention-deficit hyperactivity disorder (ADHD), and bipolar disorder. Additionally, patients with insomnia are 2 to 10 times more likely to experience new or recurrent episodes of depression.
    Insomnia is also linked to certain personality traits, such as social introversion and the repression of feelings.
    Over time, poor sleep and concerns about sleep can become associated with going to bed, resulting in a chronically poor sleep pattern that affects daytime activities.

  3. Medical Conditions and Age:

Certain medical conditions can interrupt sleep, increasing the risk of insomnia. These include chronic pain conditions, asthma, heart failure, stroke, gastrointestinal issues, and an overactive thyroid.
On the other hand, most adults over the age of 65 require about seven to nine hours of sleep per night. However, people in this age group often struggle with insomnia.

  1. Medications and Stimulants:
    There is a wide variety of prescription medications that can affect sleep patterns, and drowsiness is one of the most commonly reported side effects of medication use.
    While some medications can cause insomnia, others disrupt sleep patterns or cause daytime drowsiness.
    Additionally, over-the-counter medications for pain, allergies, cold medicines, and weight loss products may contain stimulants that contribute to insomnia. If you believe a medication may be interrupting your sleep, you should talk to your doctor. They may switch you to a different medication or adjust the dosage of your current medication.

  2. Caffeine:

Caffeine is one of the most consumed stimulants in the world!
It is most commonly found in coffee and other beverages and is specifically used to combat drowsiness and improve performance. Stimulants make it more difficult for the brain to achieve the relaxation state needed for sleep.

An important fact to know:
The amount of time it takes for the body to break down 50% of caffeine is between three and seven hours.
Larger amounts and/or repeated doses of caffeine lead to a decrease in caffeine clearance. As a result, caffeine consumption can affect sleep for many hours.

  1. Alcohol, Nicotine, and Smoking:
    While most people think of alcohol as a sedative, the reality is that it increases the release of dopamine in the brain, which has a stimulating effect. Chronic alcohol consumption is associated with insomnia.
    On the other hand, the use of nicotine replacement therapy and nicotine itself, as well as withdrawal, can contribute to insomnia. A study analyzed 29 years of data from a community study to assess the prevalence of insomnia among heavy/continuous smokers, late-onset smokers, occasional smokers, quitters/reducers, and non-smokers. The study found that chronic smokers were more likely to develop insomnia symptoms later in life.
    Exposure to secondhand smoke was also associated with insufficient sleep among people who never smoked or who had quit smoking.

  2. Stress:

People under stress often struggle with insomnia. Concerns about finances, work, school, and family problems can cause a state of hyperarousal, making it difficult to relax into a restorative sleep state.
Stress can also decrease the quality of sleep.
Major life events, such as job loss, moving, the birth of a child, the death of a loved one, or divorce, can also trigger an episode of insomnia.

  1. Shift Work:
    Almost 10% of people with a non-standard work schedule (such as rotating shifts, on-call work, or permanent night shifts) experience a disconnect between their body's circadian rhythm and the Earth's natural day-night cycle.
    An additional 30% of shift workers experience symptoms such as excessive sleepiness when they need to stay awake and alert, insomnia, focus problems, lack of energy, and depression.
    Even shift workers who sleep enough during the day may experience some of these symptoms, as their internal clock sends signals indicating it’s time to sleep during nighttime hours.
    Shift work may also be associated with a higher risk of certain diseases, such as cancer and heart disease.

Most people should aim to sleep between seven to nine hours per night. However, it is estimated that one-third of U.S. adults do not get the recommended amount of sleep.
Sleep is essential for learning and memory formation, emotional well-being, physical growth and development, immune function, and cardiovascular health. Sleep is also an integral part of cellular repair processes and helps regulate insulin and other hormones that control appetite. Sleep deficiency can dramatically decrease a person's quality of life. Insufficient sleep also increases the risk of a variety of health problems, including cardiovascular diseases, depression, and obesity.
Despite the serious consequences of insomnia, conventional treatment options are far from ideal. For example, popular hypnotic sleeping pills have consistently been associated with a significantly increased risk of death and other serious side effects.
These alarming results highlight the need for safe and effective strategies to improve sleep quality. As a recommendation, we encourage you to consider the following general sleep hygiene and lifestyle measures:

  • Keep your bedroom cool, dark, and quiet.

  • Maintain consistent sleep and wake times throughout the week.

  • Avoid eating large meals two to three hours before bedtime, as indigestion can make sleep difficult.

  • Limit stimulant intake during the day, especially close to bedtime.

  • Engage in physical activity daily.

  • Establish bedtime routines (take a warm bath, listen to soft music) to improve relaxation.

  • Opt for natural alternatives like melatonin and valerian to help improve sleep and restore the body's natural circadian rhythm.
